Assisted Living Services We assist high-risk seniors who are frail or cognitive impaired while continuously living at their own home. Bereavement Professional and supportive services to bereaved families residing in Toronto and York region. Carefirst Lifeline Live confidently and independently in your home, with the the Lifeline medical alert service. Chronic Disease Management We aim to enhance quality of life through community-based chronic disease prevention. Elder Abuse Helpline We are committed to elder abuse prevention strategies and programs to combat elder abuse. Home Care Service We help clients maintain a good quality of life and remain independent in the community. Transitional Care Centre Our 30 bed unit aims to provide an environment promoting exceptional recovery to those in need.
